Person Centered Planning (PCP)

If you are looking to get a family member involved in the Self Determination Program then this is your first step. SDP allows participants and their families the autonomy to choose what services work best to ensure that participant’s maximize their enjoyment of life. Assembling the PCP is a vital step in the SDP process and through it, the individualized goals are made clear to the participant’s entire team.

This service provides essential assistance in handling the PCP plan from inception to finalization.

PCP & Regional Center Budgets/Spending Plan

In addition to all assistance under the PCP service, this service bundle also includes assistance in corresponding with your local RC and formulating an acceptable budget plan that allows the participant to engage in all of his/her desired activities.

This service offers additional guidance on the SDP process and its unique requirements while ensuring holistic development and happiness are achieved.

*This service cost would be a percentage of the overall budget (ex. 5%)

SDP Program Management

Dedicated to providing specialized support and personalized assistance for individuals with distinct needs, enabling them to thrive in their own unique ways. This service bundle is available to those who need prolonged assistance with SDP and is a great option for first year participants.

This service aims to alleviate all worry and stress that the participant and their family may encounter by taking full control of the logistical side of things. Alongside tapping into all the resources available under the PCP & Spending Plan bundle, we will also deal with all correspondence with your local RC, collaboration with the chosen fiscal agency, and all invoice inquiries.

*This service cost would be a percentage cut of the approved overall budget (ex. 20%)

Independent Facilitator

Independent Facilitator *

One of the most common terms that you’ll see in conversations about SDP is ‘Independent Facilitator’ or IF for short. This term is the official title given to an individual who helps you write the PCP report and who can continue to offer supports throughout the year. All PCP and SDP services listed above are performed by a certified Independent Facilitator on our team!

IPP Supports

Individualized Program Plan meetings can be stressful, and at SPĒS, we’re here to help. IPP meetings aim to identify goals that the participant has, as well as traditional RC supports/services that can help them achieve those goals.

The IPP Report acts as an official agreement contract between you and the Regional Center. Due to its significance, clients and their families may sometimes need assistance. Through this service, we will help you advocate and secure traditional services that will allow you to continue to grow and succeed.
